Patient Information
Availability
Our office has early morning and evening hours available for your convenience. Saturdays are available upon request and reserved for larger and lengthier cases.
Cancellations
We ask that you give us at least 24 hours notice for any cancellations. This allows us to offer this time to another patient who is waiting for our services. We do however understand that extenuating circumstances may occur.
First Visit
Before scheduling your first visit please let us know if you have recent dental x-rays for us to request from your previous dentist, if you will be bringing x-rays, and if you need to pre-medicate before your dental visits. If you have dental insurance, please bring your insurance card or claim form so that we may access your insurance and give you a break down of your benefit coverage.
If you are under 18 years of age, you must be accompanied by a parent or guardian.
Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
Fees
Our fees are set by the usual and customary fee schedules in our area.
Dental Insurance
As a courtesy to our patients, we process your insurance claims electronically. We ask that you pay “your portion” at the time of service. Patients without insurance are expected to pay in full at the time of service unless other financial arrangements have been made in advance.
HIPAA/Privacy Policy
On April 14, 2003, the Health Insurance Portability and Accountability Act went into effect. Our office complies with the HIPAA regulations and we ensure the privacy and confidentiality of each patient. Our notice of privacy practices is posted in our waiting room and is given to every new patient for review. We ask that you sign a written acknowledgment after reviewing our office policy.
